MONTHLY CONSISTENCY REWARDS PROGRAM

With consistency comes change. Embody Wellness believes lasting change comes with consistency, our bodies adapt to what we consistently do and our modern world is not designed in a way that promotes ideal movement patterns. We want to reward your commitment to yourself.

Agreement Terms:​

With the Monthly Consistency Rewards Program, you will receive 1 massage + 15% off time upgrades to equal a maximum of 90-minutes + 1 free $5 add-on + 1 free restorative fitness class every 4-weeks for a 12-week commitment.

 

This commitment is for 12-weeks from the date of purchase and will automatically renew every 12-weeks. (see exceptions below)

Agreement to Recurring Payment: 

As a Consistency Rewards Program Member, you agree to pay Embody Wellness a recurring payment every 4-weeks of $95/$115/$135 for the duration of 12-weeks. Once the terms of this agreement are satisfied, the Monthly Consistency Rewards Program will automatically renew for an additional 12-weeks until canceled in writing. All cancellations must be received in writing 30-days prior to your next 12-week billing cycle.

Cancellation Policy:

​

Appointment Cancellation: 

If you need to cancel an appointment or reschedule an appointment, you must notify the office 24-hours prior to your scheduled appointment. If you fail to give 24-hour notice of canceling a scheduled appointment, you will have 3 options; you can pay a $50 cancellation fee and retain your session, you can have someone go in your place, or you can choose to forfeit that month's session. If you choose to book an additional session that month to replace your missed appointment, we will honor your member rate of $90 for that additional appointment.

 

No Show No Call:

If you fail to call or do not arrive for your scheduled appointment, you will automatically forfeit your session for that month. If you choose to book an additional session that month to replace your missed appointment, we will honor your member rate of $90/$110/$125 (60/75/90-Min) for that additional appointment.

 

Early Program Cancellation:

If you wish to cancel your Consistency Rewards Program prior to the end of your current 12-week billing cycle, you must notify us in writing or by emailing us at embodywellnessventura@gmail.com 10-days prior to your next recurring payment and you will be charged a $50 early cancellation fee.  

 

Pausing Your Rewards Program:

To pause your Consistency Rewards Program, you must submit a request in writing or by emailing embodywellnessventura@gmail.com 10-days prior to your next billing date for your request to be effective for the following month. Members may elect to pause their Consistency Rewards Program for a maximum of 60-days, two times per year. Following the end of your requested account pause period, your monthly auto-renew will automatically be reactivated.

Permanent Relocation:

You can cancel this agreement should a permanent relocation of your residence take place, making it difficult to keep your appointments. Proof of residence and written notice are required 30-days prior to the next billing month. Your new residence must be outside of a 15 Mile radius from the Embody Wellness Office.

 

Availability Restrictions

Therapist availability is subject to change and this agreement is not dependent on a specific therapist's availability. We have a well-trained, talented staff of therapists and are confident we can find a great fit for you. If your preferred therapist is unavailable, we are happy to make a recommendation for an alternative therapist. We understand that we all get comfortable with "our therapist" but every therapist brings a unique style to this work and a new therapist will learn your body just as quickly as your other therapist. We encourage everyone to have at least two go-to therapists.

Using an Allotted Session Prior to your Original Autopay Schedule

If you book your allotted session prior to your next autopay date, we will adjust the date of the upcoming payment to the date you come in for your session and future payments will remain on the original payment schedule.

Unused Sessions

Unused massage sessions expire 1 year from the payment date and can be used at any time within that year or until you cancel your Consistency Rewards Program according to the cancellation terms above. At the time of cancellation, any active unused sessions will be cashed out for Gift Vouchers that you can use or gift to someone else. These Gift Vouchers will be good for 90 days. You may only keep up to 3 unused sessions on your account at any time and any unused session in excess of 3 will be cashed out in Gift Vouchers.

Unused class sessions expire 30-days from the payment date.

Unused add-ons expire 30-days from the payment date.

By purchasing this rewards program, you agree to the following:

(a) I agree to purchase the Monthly Consistency Rewards Program for a recurring 12-week commitment at $95/$115/$135 every 4-weeks as an automatic charge to my credit card account. (b) I understand autopay will continue with another 12-week commitment at the discounted price and all terms & conditions will remain valid until written notice is given at least 30 days prior to the next billing cycle; (c) I hereby certify that I am the holder of the credit card attached to my account and that it is valid and current; (d) I understand that I will be notified if my credit card payment fails to authorize for any reason, and that a $15 late fee will apply if I do not provide a valid credit card within 10 calendar days of the original rejection date; (e) I understand that my service will be deactivated if my account becomes more than 30 calendar days late; (f) I understand that I am still subject to late cancel and no show fees if I don't honor current cancellation policies, which are an extra fee outside of the $95/$115/$135 every 4-weeks rate; and (g) I understand that this is the rate I will keep as long as I choose to continue this program or as long as Embody Wellness offers this program. If I decide to cancel and then later return as a member, I'll be subject to the current rates. (h) I understand that Embody Wellness has the right to refuse service to anyone and can cancel my contract without notice, and I understand I will not receive a refund for my unused sessions. (i) Terms of this contract are subject to change without notice.
